Registering with Penrose Surgery
New patients living in our catchment area are welcome to join.
If you wish to register with us online click on the link below to open the form. When you have completed all of the details, click on the “Send” button to mail your form to us. You will then need to visit the surgery where you will be asked to provide proof of address, photo ID and signature. You will then need to book an appointment with the Health Care Assistant for a new patient health check to complete your registration. Please note that children aged 16 and under do not need to complete the registration form. An adult member of the family can register them by providing the required information and signing the form on their behalf.
Documents to bring with you when completing registration:
- Photo ID (passport/driving licence)
- Proof of address in catchment area, i.e. current utility bill
- For children (under 5 years of age): red baby book
- For children (birth – 18 years if no photo ID available): birth certificate
Change of details
It is important that you inform us of any changes in your name and address or phone number so that we can contact you when required.
If you move outside the practice area you may be asked to find another practice.

If for any reasons we are unable to process your registration, you may contact the Practice Manager or Patient Advice & Liaisons Services (PALS). Details of other primary medical services in the area may also be obtained through PALS 0800 587 7027.
New or existing patients from outside our catchment area are not eligible for home visits and may be deregistered and advised to register with another GP locally if they are unable to attend the appointments on time or if their health needs change such that home visits are required.
